Please use this portal to upload documents that are  currently not electronic form submissions.  Documents must be in a PDF format.  Images will not be accepted by the file upload.    This portal is for advisor and student use.  All forms must be complete and signed by all approvers or the form will be returned.

1. The form MUST be signed by the advisor
2. You MUST go through your department to obtain the CRN and other requirements of your Undergraduate Research.
3. Your form will be declined  if you do not have the form processed according to the requirements of your department.
4. Many forms are processed at the department level.  TA's, Peer Mentor's, Field Study, etc.. are not processed by CALS AP.
